The Women's and Children's Hospital (WCH) recognises that not all patients are the same and some require higher levels of support.
For many families the hospital environment can be confusing and overwhelming. At the same time they are adjusting to life with a child who has high health needs.
The Complex Care Coordination Unit (CCCU) includes a Nurse Consultant and Social Worker who can provide support for children with high health care needs and their families. The staff in the CCCU can offer a single point of contact for advice and help when coordinating health and community services.
The Complex Care Nurse Consultant and/or Social Worker work closely with relevant healthcare professionals to help facilitate and co-ordinate services within the hospital and community settings.

The Complex Care Nurse Consultant is able to assist families of children who meet all of the following criteria:
The referral to the Complex Care Nurse Consultant is reviewed every 12 months.
This support is offered for children who have a General Paediatrician involved in coordinating their care. If a Nurse Consultant is part of the referring team, then the roles of the speciality Nurse Consultant and the Complex Care Nurse Consultant will need to be negotiated.
Families can be referred to the Complex Care Nurse Consultant by health care professionals including medical, nursing and allied health staff.
